Over the past 20 years, Jeremy has witnessed patients get caught in a vicious cycle of pain, doctor visits, tests, specialists, and devices– with limited results. The cost– both metaphorically and financially– contributes to patient fatigue, hopelessness and distress. 
Observing this motivated Jeremy to create a care model centered around the patient– not only focused on their current state but also taking information from their histories and goals for future plans for physical health into account. The ultimate goal of this Values Based Care model is to ensure the patient receives the most effective and efficient care possible.
